An Established Patient is one that has been
seen in our office within the last three years.
Scheduled time with our professional staff is considered to be extremely valuable. They have set aside their time to be available to you. Therefore, if you find it necessary to cancel or reschedule your appointment, please contact our office as soon as possible. Thank you.
Please bring the following with you when you come for your appointment:
1) A valid referral, if required by your insurance company. Please ask your PCP's office for a printed copy of this referral,
even if they say that it is "in the system".
2) Your insurance information. Please bring your card(s) with you so we can scan them for your records.
3) Your co-payment.
4) The complete name, address and telephone number of your primary care physician. The fax number would also be helpful,
if you have it.
5) A list of all your current medications, including those that are over the counter.
6) A list of any allergies to medication.
7) The name, address and telephone number of your pharmacy.
9) A photo ID for all patients.
10) Fill out the New Patient Registration Form. It is available here online for anyone to download and fill out at their own leisure if they feel more comfortable doing it at home rather than filling it out at the office. When completed, please hand it in with the rest of the requirements on your next visit.
